Information requirements are set by our governing league and Pop Warner.
Portage Pop Warner is accountable to its parent organizations, and we require participants to complete and submit forms exactly as directed before being allowed to participate.

2025 POP WARNER Participant Contract & Parental Consent Form
REQUIRED.
All signatures and dates must be completed.

2025 SCHOLASTIC DOCUMENTATION 
REQUIRED.

CURRENT, FULL-YEAR REPORT CARD. DO NOT SEND POWER SCHOOL REPORT. 
REPORT CARD MUST HAVE THE FULL SCHOOL YEAR OF GRADES FOR 2024-2025
If need a copy of report card, one may be requested by parent or guardian at:

Birth Certificate
REQUIRED.


ALL  PARTICIPANTS:
AN ORIGINAL BIRTH CERTIFICATE WILL BE REQUIRED TO BE PRESENTED TO A DISTRICT HLA MEMBER  PRIOR TO POST SEASON GAMES TO VALIDATE OUR COPY ON FILE.
Please ensure you have a valid ORIGINAL Birth certificate available.

Please do not send an original, but ensure the copy is of an official birth certificate.
A "Mother's Copy" is NOT a birth certificate.
Birth certificates are available from the county/state of birth.
